The voting machine miscount (Dominion Voting Systems?) happened in one batch due to a misconfiguration, not fraud. It was discovered and corrected, the machines themselves worked fine. Yes, it can happen elsewhere, too. Automatic counting is problematic. That's what recounts by hand are for.
